Quantifying to Decide: New Imaging Biomarkers

Moderated by Prof. Jean-Pierre Pruvo, this session brings together two neuroradiologists practicing in France and in the United States, Dr. Charles Mellerio and Prof. Lotfi Hacein-Bey, around a simple question: today, in MRI, what do we really know how to measure, and what actually helps in making decisions?

Three parts to answer it. First, the tumor. A case managed “the old-fashioned way,” from the first CT scan all the way to the operating room, serves as the starting point. Each step is then revisited with what quantification changes: perfusion for neoangiogenesis, fMRI before surgery, volumetry for follow-up, and APT-CEST when a decision must be made between pseudoprogression, radionecrosis and recurrence. Next, vascular and neurodegenerative disease, from what is already in place, with emergency stroke triage and microbleeds on SWI, to what is coming: cerebral oxygenation (OEF, CMRO2) and REACT, which adds information on receptors to fMRI and opens avenues as far as psychiatry. Finally, a before/after review of both parts, to sort out what can be used right now in one’s department and what still belongs to research.

Can We Do Without Contrast Agents in MRI Neuroimaging?

Moderated by Prof. Vincent Dousset, this round table brings together four neuroradiologists practicing in France, Italy, and the United States to compare their approaches to a question that has become part of everyday practice: how far can we reduce contrast agent injection in brain MRI without losing diagnostic information?

Three stages to address the question. First, the real impacts of gadolinium — biological, economic, and environmental — from production costs to accumulation in the body, without forgetting its integration into the technical fee. Then, the levers for more rational use: what information does contrast injection really provide, indication by indication, and where can doses be reduced or injections spaced further apart in the follow-up of MS, schwannomas, macroadenomas, and glial tumors? Finally, the alternatives currently available, from CEST to ASL, through new agents and AI-based extrapolation, with an open discussion on forthcoming recommendations from the SFNR and SFR.

⇒ A practical, cross-disciplinary discussion format, providing concrete guidance that can be applied in everyday practice.
